Nature of the assignment

The provision of data is optional for you, but the owner announces that the possible non-disclosure of one of the mandatory information, has as a consequence the impossibility for ESSEBI MEDICAL SRL to carry out the same commercial / contractual relationships or, in case of incorrect communication of that information, the impossibility of the holder to ensure the adequacy of the treatment itself to the contractual agreements for which it is performed or the possible failure to match the results of the treatment with the obligations imposed by the tax, administrative or labor regulations to which it is addressed.You can assert your rights as expressed in the art. 7, 8, 9 and 10 of Legislative Decree 30 June 2003 no. 196, by contacting the holder (or the manager, if appointed) of the treatment.The data controller is ESSEBI MEDICAL SRL with registered office in Faetano (Republic of San Marino) - Economic Operator Code SM 26889, in the person of its legal representative.
